The Quality and Productivity challenge is paramount in delivering high quality and sustainable care for people with Long Term Conditions. As Long Term Conditions account for approximately 70% of the health and social care spend in England, it is essential that care is delivered in the right place, by the right people to reduce admissions and improve the patient experience. For long term conditions this means:
- Making integrated services a reality. Work across organisational boundaries and overcome cultural differences between organisations to deliver seamless care to people with long term conditions, maximising your human and capital resources
- Reduce costs and better manage your resources by promoting Self Care. Understand how best to support people to self manage their conditions.
- Move care closer to home. Ensure people are able to access care locally and feel supported at home and in their community to improve health and reduce hospital admissions.
- Tackle both the strategic and operational challenges in: Respiratory Conditions, Diabetes, Cancer, Heart and Cardiac, Musculoskeletal Conditions, Neurological Conditions and Mental Health
Attend MLTC 2010 to learn from the experts how to make these goals a reality. Plus - hear from Sir John Oldham, the new National Clinical Lead for Quality and Productivity. In this new role, he is the Department of Health's lead on long term conditions, urgent care and the integration between health and social care.Register as a conference delegate to gain full access to the CPD certified conference streams, exhibition village, poster presentations, complimentary tea, coffee and hot lunch, and a range of visitor attractions including seminars and clinical sessions.
